  • Pelvic floor therapy is a therapeutic service speciality provided by occupational therapists or physical therapists. We evaluate and address dysfunction related to bowel health, bladder health, and sexual health. Here at Kula Wellness, we use a whole person approach to determine how the mind and body influence one another and address the underlying cause of dysfunction. We also provide services for the pregnant and postpartum population such as birth preparation, postpartum recovery, and diastasis recti evaluation and treatment.

  • At Kula Wellness, we treat persons born female ages 18+.

  • We are out of network with insurance companies. We will provide a SuperBill for reimbursement. It is advised that you contact your insurance company in order to fully understand your out of network benefits and reimbursement (see below for more information)

    Providing out of network services enables quality 1:1 care without the overarching concern of “treating for insurance”. Transparency is important to us. With our cash-based services, clients are provided the price up front without any hidden costs or unexpected bills.

  • It is recommended that you call your insurance company to get information on your specific benefits to avoid unexpected costs after receiving therapy.  

    Using the customer service number on the back of your insurance card, here is a quick overview of questions to ask prior to starting pelvic floor therapy.

    • What is my deductible? And will services only be reimbursed after I meet my deductible? 

    • Is a pre-authorization required? 

    • Is a referral from a doctor required for reimbursement of outpatient PT/OT services? 

    • What percentage of reimbursement do I have? And is there a cap after a certain amount? (for example, 60% reimbursement rate for up to $100) 

    • Are there any special forms to be filled out prior to reimbursement? 

    • Where do I submit my “SuperBill” to?

    Payment will be collected at the time of each session via cash, credit card, check, and or FSA/HSA. It is your responsibility to understand your insurance benefits and submit the provided SuperBill accordingly.

  • No, a SuperBill is not my amazing husband— he’s Billy to me.

    A SuperBill is a receipt provided at the end of each session with diagnosis codes and treatment codes that can be submitted to your insurance company for reimbursement. Please contact your insurance company directly for the steps to submit a SuperBill.

  • Yes there is a parking lot available for clients, which is quite amazing in Montclair #iykyk

  • Yes, there is an elevator entrance off of the parking lot, and an elevator to the third floor. There are accessible bathrooms on the first floor.
